Job Summary:

To report to the Transportation Supervisor, this position is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.

Effectively communicates with participants, families, staff and other agencies and departments.

Drivers must have phone communication turn on during normal work hours.

Drives a specially equipped van to deliver physically limited persons to varied destinations. Ensures that wheelchair lifts and tie down equipment are properly operated and that participants are delivered to appointments in a timely manner.

Follows established schedules, adjusts schedules to meet unanticipated changes, determines the safest and most efficient pick-up and delivery routes.

Assists passengers on and off the van in a safe and courteous manner. Deals with medical equipment and other emergencies that may arise.

Inspects van and maintains it in a clean, orderly and safe condition. Reports the need for maintenance.

Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any change in participant condition to team members.

Completes documentation as required.

Performs other duties as assigned.


Education: High school diploma, GED or equivalent preferred

Experience: At least 1 year experience in transporting handicapped individuals. Must have 1 year experience working with the frail or elderly.

Number and Type of Employees Supervised: None

Licensure, Registry or Certification Required: Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.

Special Training: Must be able to work effectively in a team environment and operate handicap accessible vehicles. Driving is required for this position using a Prisma Health vehicle. Employee will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies. Employee must pass driver training.
